Medisafe alternatives 2026: check your account first
The two-medication cap everyone cites is a non-US change from late 2024. Medisafe's own terms say something narrower. Check yours in 90 seconds, then switch or don't.

If you're in the US, open Medisafe and add a third medication before you switch to anything. The two-medication cap filling search results is a change that landed outside the United States in late 2024, and Medisafe's own terms describe something much narrower than the posts do. Takes 90 seconds to find out which applies to you.
We haven't lab-tested these apps. This verdict comes from Medisafe's published terms, its US App Store listing, and the sources the competing posts do and don't cite. Our methodology covers how we check. We sell nothing and earn nothing here.
What the primary sources actually say
Medisafe's terms and conditions, last updated December 1, 2025, contain exactly one dated commitment:
"Beginning January 1, 2026, continued access to the Service in certain countries outside the United States will require an active paid subscription"
Outside the United States. There is no medication count limit anywhere in that document.
Meanwhile the US App Store listing still shows the app as free with in-app purchases, sitting at 4.7 stars across 101,000 US ratings, with premium tiers disclosed at $2.99, $4.99 and $9.99 monthly and $27.99, $29.95 and $39.99 yearly.

The cap is real. It's just not the story you were told
Dig past the blog posts into user reports and a different sequence appears. The two-medication restriction shows up in late 2024, priced at 43€ a year, in reviews written by European users.
Euros. Not dollars. And 2024, not 2026.
That lines up exactly with what Medisafe's terms describe: a staged non-US tightening, with the January 2026 date marking when certain countries outside the US moved to requiring payment outright. Two separate regional changes, roughly a year apart.
The posts on page one collapse both into a single headline aimed at American readers: Medisafe is not free anymore, as of January 2026. For a US reader that is, on the available evidence, wrong.
The sources can't even agree with each other on when the cap started. October 2024, November 15, December 2. Three dates for one event is what copying looks like.
The $59.88 tell
One detail gives away that nobody checked.
Every post quoting a yearly price for Medisafe Premium lists $59.88. That number appears nowhere in Medisafe's App Store listing. It's $4.99 multiplied by twelve.
The actual disclosed yearly tiers are $27.99, $29.95 and $39.99. So the figure being repeated as Medisafe's annual cost overstates the cheapest real one by 114%, and the dearest by 50%. Anyone who had opened the listing would have caught it.
The most-cited of these posts is published by Pillo, which sells a competing medication reminder app, and cites no Medisafe documentation, no App Store listing, and no announcement. Of the seven results on page one for this query, five are published by companies selling an alternative.
That doesn't make them liars. People do get paywalled, and non-US users are getting paywalled right now. It makes them unreliable narrators about a competitor's US pricing.
The 90-second check
Skip the debate and query your own account:
- Open Medisafe and add a third medication.
- If it saves, the cap doesn't apply to you. Stop reading, keep your app.
- If you hit a paywall, note the currency on the payment screen. It tells you which regional regime you're in.

If you do want out
Three real options, none of which require the cap story to be true.
MyTherapy is the one I'd move to. Free, 4.8 stars across 8,300 US ratings, and the paid tier is honest about what it sells: $1.99/month or $14.99/year removes ads, with a $49.99 one-time Plus purchase. No medication cap in the listing, no countdown to a paywall. It does refill tracking and health reports Medisafe charges for.
iOS 26.2's Urgent reminders cost nothing and beat both apps on the only thing that decides whether the pill gets taken: they ring through Silent mode and Focus. We went through the setup in our medication reminder comparison. The catch is that Reminders doesn't track adherence, refills, or interactions. It nags. Sometimes that's the whole job.
A $19–30 timer cap or pill organizer solves a different failure. If your actual problem is "did I already take it," a physical object that shows the answer beats any notification. Covered in our pill organizer breakdown.
What Medisafe still does better
An honest ledger, since most pages about this are written by its competitors.
Medisafe's drug interaction warnings, refill tracking, and the "Medfriend" feature that pings a chosen person when you miss a critical dose have no free equivalent in MyTherapy or Reminders. 101,000 US ratings at 4.7 stars is not an app people hate. If you take several interacting medications and someone else helps you stay on top of them, Medisafe at $27.99 a year is defensible, and it's cheaper than four of the six ADHD planner apps we track in what ADHD apps cost.
Where it genuinely fails: the free tier is narrower than it was, non-US users are being charged, and the company has communicated all of this badly enough that its competitors got to write the story first.
Don't switch apps because a rival's blog told you to. Switch because you opened yours and didn't like what you saw.
